4. Average and median processing time from submission to final determination, by year, further broken down by:

5. Number of employees who were terminated, laid off, demoted, or non-renewed within five years of submitting an ADA request—broken down by year, department, and classification (e.g., classified staff, OA).
6. Number of ADA requests that:
• Were escalated or appealed internally
• Led to complaints with HR, OICRC, BOLI, OCR, EEOC, or other agencies
7. Average and maximum time between notifying a supervisor of an accommodation need and formal submission to the ADA Office. If not tracked, please confirm.
