I'd like to request additional documents that were mentioned in the Feb. 27 investigation into Delta Sigma Phi. There were numerous documents listed to be in the appendix that were not included in the files released to me. I'd like to request the Nov. 3 Community Care and Support Form, the Nov. 20 Notice of Allegations and the Nov. 8 UOPD Contact Contact/Records Form.
