I am requesting COVID-19 vaccine exemption numbers and relevant data for the fall term, under Oregon public records law.

I would like to receive:

• The total number of COVID-19 vaccine exemptions (separate totals for students and employees)
• The total number of COVID-19 vaccine exemptions broken out into the types of exemption (religious, medical, philosophical, etc.) for students and employees
• The total number of students and staff members who did not attest their vaccination status or request an exemption (i.e. the number of students and staff who did not comply with the university’s vaccination requirements)
