Pursuant to the Oregon open records law, ORS 192.410 to 192.505, I write to request a copy of the reported concussions for the University of Oregon football team from 2007 to 2015.  

Format: we prefer to receive records in the following formats, listed in order of preference:

1)    an electronic data format such as a .csv, spreadsheet, delimited data set, or similar;

2)    other non-proprietary electronic format;

3)    word processing file, text-based PDF, or similar;

4)    paper copies.  

We realize some of this information requested may include personally identifiable content about students that would violate HIPAA and FERPA law. We will accept a document that redacts this information.
