Any secondary or level III violations, interpretations, waiver requests, or student-athlete reinstatement requests regarding student-athlete self-employment, promotional activities, charitable activities, or use of student-athlete name, image, or likeness, since Jan. 1, 2018.

Therein, I request any such documents that reference NCAA Bylaws 12.4, 12.5 and/or their subparts, or any of the following keywords: “self-employment,” “promotional activities,” “charitable activities,” or “name, image or likeness.”
