Pursuant to the Oregon Public Records Law, I am requesting the following public records to be
transmitted electronically via email or via a confidential file transmission system:
1. If applicable, (1) interim suspension notices, (2) notice of investigations, (3) notice of
violation (charge letter) with the appropriate incident report or investigative report, (4)
case election form (how the representative chooses to resolve the case), (5) outcome
letter, ( 6) submitted appeal materials by the organization at all available levels of appeal,
(7) any responses to those appeals, {8) appeal decision letters at all available levels of
appeal, and (9) if no appeal was submitted, the final notice of discipline sent to the
organization, for the following organization conduct proceedings:
