Please consider this a public records request for copies of the following records in the possession of your institution's athletic department since 7/1/22: - Blank versions of any and all forms that incoming, continuing and transfer student-athletes complete annually to disclose any conduct that resulted in discipline through a Title IX proceeding, a criminal conviction, or, for transfer students, whether a Title IX proceeding was incomplete at the time of transfer. - Any and all written procedures directing staff to gather information that reasonably yields information from an incoming or transfer student-athlete's former institution(s) about whether the student left the institution with an incomplete Title IX proceeding, was disciplined through a Title IX proceeding or has a criminal conviction for sexual, interpersonal or other acts of violence. - Any and all written policies/procedures/protocols outlining the specific steps your institution takes to check the backgrounds of incoming or transfer student-athletes to determine if they left their previous institutions with an incomplete Title IX proceeding, were disciplined through a Title IX proceeding, or have a criminal conviction for sexual, interpersonal or other acts of violence. - Any and all written athletic department policies/rules outlining the types of conductthat can result in an incoming, continuing or transfer student-athlete's disqualification from competition, practice or athletic aid. (For example, a felony conviction, a finding of fault in Title IX proceeding, etc.)
