I am writing to request a summary of information about all currently active personal service contracts of the University of Oregon.  

Specifically, what I request is a spreadsheet which contains, at a minimum, the following information for each contract:

• Award Title
• Award Type
• Contractor Name
• Contractor Address
• Start Date of Contract, and dates of any amendments
• End Date of Contract
• Orignal Amount of Contract
• Amount of Amendment(s)
• Total Amount of Contract as Amended
