I am requesting an electronic copy of any document Oregon student-athletes are being asked to sign pertaining to COVID-19, either by UO athletics and/or UO general council. This includes but is not limited to any liability waiver, letter of indemnification, pledge, honor code, written declaration, agreement, memorandum of understanding, contract, statement or other document signed by UO student-athletes and/or their parents related to COVID-19. To be clear, this is not a request for any document signed by a UO student-athlete or anything identifiable or covered by FERPA, only the blank document.
