I am requesting access to the aggregate number of painkiller injections and painkiller prescriptions provided to football players (students athletes) under Oregon’s Athletic Department during the last two years: 2009-2010 and 2010-2011. I understand that student athlete personal information such as name, Social Security Number and home addresses are protected by privacy laws, and that any data which fall under this request which contains such information will be redacted. Among the PAINKILLER INJECTIONS about which we seek information are: • Toradol (Brand Name) or Ketorolac (Generic Name) • Marcaine (Brand Name) or Bupivacaine (Generic Name) • And any other analgesic, narcotic or other painkiller injections given to student athletes PAINKILLER PRESCRIPTIONS about which we seek information include: • OxyContin (Brand Name), Percocet (Brand Name) or other varieties of Oxycodone (Generic Name) • Vicodin (Brand Name), Norco (Brand Name) or other varieties of Hydrocodone (Generic Name) • Oxymorphone • Hydromorphone • Morphine • Codeine • Fentanyl • And any other painkiller prescriptions given to student athletes
