Please consider this a public records request for blank copies of any transfer tracer forms sent by your institution's athletic department to other institutions since Jan. 1, 2014. One blank copy is sufficient, unless your institution has used multiple versions of its transfer tracer form during that time period, in which case I'd like one copy of each one.
