1. A copy of all Student-Athlete Employer Forms submitted by student-athletes listing their sport as Football or Men’s Basketball for the 2017 summer vacation period.

A copy of all Student-Athlete Employment Approval Forms submitted by student-athletes listing their sport as Football or Men’s Basketball for the most recent Fall Quarter, Winter break, Winter Quarter, Spring Quarter, Spring Break, and Summer break/session.  Please redact the Student-Athlete Name, Phone, Email address, and Student-Athlete Signature from the forms."
