   1. Current employment contract and salary for 2015-2016 academic year (Letter of intent, form agreement, or applicable document / record which shows annual compensation and start/end date of employment).  If an employee does not have a formal written agreement, please provide (ideally in a spreadsheet or table format): Name, Compensation, and Start/End Date (or write “At Will” if applicable).
   2. Most recent outside income form (ideally 2014-2015 fiscal year) for any employee if available (in cases where such a record exists per NCAA bylaw 11.2.2).
   3. Most recent income (ideally 2014-2015 fiscal year) associated with Sports Camps for any employee, if available.

2. Pursuant to applicable authority, I am writing to request copies of the following “Documents” for [13 athletic department staff]:
   1. Most recent outside income form (ideally 2014-2015 fiscal year) for any employee if available (in cases where such a record exists per NCAA bylaw 11.2.2).
   2. Most recent income (ideally 2014-2015 fiscal year) associated with Sports Camps for any employee, if available.

3. Pursuant to applicable authority, I am writing to request a copy of any contract or written agreement in force (i.e., signed, fully executed) between the University’s Department of Athletics (or its affiliates) and third parties regarding all of the following categories of subject matter:
   1. The current Outsourced Ticketing contract I have expired.
   2. Video management (through firms such as XOS).
   3. Multi-media rights (through firms such as IMG, Learfield, Nelligan, etc).
   4. Concessions for all athletic facilities (through firms such as Aramark, etc).
   5. The current agreement I have previously received with Nike is set to expire in May 31, 2018. If there is an updated contract or amendment, please send, otherwise no need to resend.
   6. The current agreement I have previously received with Willamette Beverage Company is set to expire in June 30, 2022. If there is an updated contract or amendment, please send, otherwise no need to resend.
   7. The current Ticketing contract I have expired.
   8. Trademark licensing (through firms such as Collegiate Licensing Co.).
