Pursuant to Oregon Public Records Law, I hereby request copies of the following documents from the University of Oregon (the "University"):

1. All contracts or agreements between the University and Stephen L. Beckley,

Stephen L. Beckley &Associates, Inc., Hodgkins Beckley Consulting, LLC, and

Lookout Mountain Group (collectively "Beckley") relating to student health,

including without limitation any engagement letter or professional service

agreements between the University and Beckley;

2. All documents showing the scope of work for any services that Beckley or its

subcontractors rendered for the University relating to student health;

3. All documents Beckley submitted to the University in response to a request for proposal from the University for professional services, including without limitation the provision of consulting or actuarial services relating to student health;

4. All documents the University received from Beckley relating to Beckley's

purported abilities, qualifications, and/or expertise in providing consulting or

actuarial services relating to student health;

5. Documents sufficient to identify the Beckley personnel and subcontractors who

performed services for the University relating to student health;

6. All reports or recommendations Beckley or its subcontractors provided to the

University relating to student health regarding whether the University should

consolidate or self-fund any student health plan(s);

7. All risk warnings Beckley or its subcontractors provided to the University relating to student health;

8. All actuarial reports Beckley or its subcontractors provided to the University relating to student health; and

9. Documents sufficient to identify trend rates used or recommended by Beckley

or its subcontractors to set premium rates or determine the cost of coverage for

any student health plan.
