I hereby request ELECTRONIC copies of the following records on behalf of Sporting News:1) Any in-use procedural document that indicates the University of Oregon must note on a transcript when it finds a student responsible for sexual misconduct or sexual assault via a Title IX investigation. This record is most likely to be found in conjunction with the Office of the Registrar.  2) Any in-use procedural document that indicates the University of Oregon must note on a transcript when a student under an ongoing Title IX investigation for sexual misconduct or sexual assault leaves the University of Oregon. This record is most likely to be found in conjunction with the Office of the Registrar. 3) Any procedural document, email memo or other type of memo since January 1, 2014 that offers guidance as to whether the University of Oregon should ever under any circumstance make any transcript notation or transcript insert. This record is most likely to be found in conjunction with the Office of the Registrar, though like the previous requests it should not be limited to that office.
