Here are the documents we would like to formally request regarding UO Football Recruiting. Your electronic submission site is currently down, so I’m doing it via email.
1) The 2010 National High School Evaluation Booklet (this is the most important one, and we’d like to get it as soon as possible).

Any and all invoices and records of payment paid by the University to Willie J. Lyles in the fiscal year 2010. The expenditure report for the University maintained at the state of Oregon's web site lists a payment of $25,000 to "Willie J. Lyles/dba Complete Scouting." It is listed under the category "Books, Publications and other References."
3) Any and all invoices and records of payment paid by the University to "New Level Athletics" in the fiscal year 2010
4) Any and all documents or videos received by the Oregon football program from Complete Scouting Services from Feb. 1, 2010-March 4, 2011. This should include, but is not limited to the products Oregon received for its payment of $25,000 (dated March 29, 2010) to Complete Scouting Services for the "2011 National Package"
5) Any and documents or videos received by the Oregon football program from New Level Athletics for its payment of $3,745 to New Level Athletics which the University paid Sept. 9,2009
6) copy of University of Oregon Athletic Department purchase order UP0068816 dated March 24, 2010 to Willie J. Lyles/dba Complete Scouting Services; the invoice from Complete Scouting Services to the Athletic Department dated February 22, 2010; the invoice from New Level Athletics to the Athletic Department dated August 12, 2009; and any related documents to or from Complete Scouting Services and/or New Level Athletics
7) Any other invoices, checks or other payment/cashing information as well as tax documents - dating to 2006 - between UO and the following: a) Willie J Lyles; b)Elite Scouting Services; c) Complete Scouting Services; d) Baron Flenory; e) New Level Athletics
8) All e-mails between UO compliance office between or among the following - Chip Kelly; athletic directors Pat Kilkenny, Mike Bellotti, Rob Mullens - related to Elite Scouting Services and/or Willie Lyles and/or Lyles' Complete Scouting Services.
9) Any travel expenses incurred by Willie J Lyles paid by UO since 2007
10) A record of all correspondence with the NCAA related to Willie J Lyles involvement with UO
11) Any itinerary information related to the recruiting visits of Seastrunk and James
12) Copies of any. documentation sent to head football coach Chip Kelly from Elite Scouting Services or any law firms representing Elite Scouting Services from April 1, 2010 through August 31, 2010
13) Records of any and all payments to William (aka, Willie, Willy, Will, Bill) lyles by the University of Oregon. This includes, but is not limited to, the work Mr. lyles performed at camps or football events held at (or affiliated with) the University of Oregon or affiliated with any of its coaches
